Arthroscopic Meniscectomy


Arthroscopic meniscectomy is preferred when torn meniscus tissue is not suitable for repair. Meniscectomy is a closed surgery to remove the torn part of the meniscus. In meniscectomy, the damaged meniscus tissue is removed, leaving as much healthy meniscus as possible. After a meniscectomy, patients usually show a rapid recovery and return to their daily lives in a short time.

 

Meniscus Repair


Meniscus tissue is an extremely important structure for the knee joint and has an important role in knee joint biomechanics. After meniscal injuries, unlike previous practises, the meniscus should be preserved and anatomically repaired.

 

Meniscus repair is a surgical procedure that involves suturing the torn meniscus. Meniscus repair allows the knee to regain its normal anatomy and yields good long-term results when surgery is successful. However, meniscus repair is an important operation and when compared to meniscectomy, the time to return to daily life is slightly longer.


Meniscus Transplant


Meniscus transplantation can be considered as the only treatment option in order to prevent calcification in cases where patients lose all/nearly of the meniscus tissue in surgical/non-surgical ways. 

 

In the meniscus transplantation procedure, frozen meniscus tissues taken from cadavers are transferred to the patient after the necessary tests are performed. If the patient is suitable, the surgery is performed closed or with very small incisions.

 

In fact, maximum preservation of tissues is very important in the surgery of meniscal tears. However, In many countries, surgical removal of the damaged part of the meniscus is preferred instead of repair in cases where meniscus repair is possible, largely due to lack of surgical experience. Meniscus transplantation is applied by surgeons in the future after this type of surgery.


Limb Lengthening Surgery


Limb lengthening surgery is a surgical operation performed by orthopedic surgeon for the purpose of extending the bones in the arms and legs.

 

Apart from defects caused by leg length discrepancy (LLD), bone fractures, congenital defects and developmental disorders, it can be performed only for aesthetic purposes. It is possible leg lengthening is 7-10 cm in leg bones. One of the latest technologies of lengthening surgery is the internal method in which an implant placed in the bone is controlled externally by electromagnetic remote control.

 

The intramedullary limb lengthening system that we use in our centre allows a nail placed in the bone cavity to be controlled externally. Thus, there is no need for an apparatus that are placed outside the leg and limit movement. We offer a customized, minimally-invasive approach to limb lengthening with the less pain and complications, faster recovery, and no risk of pin-site infection.

 

Revision Knee/Hip Replacement

Revision surgery for the knee/hip was performed to replace the failed prosthesis with a new prosthetic component. Revision surgery is required when artificial joint wears out earlier than expected or implant defects. Serious complications can also lead to revision surgery.

The surgery improves mobility and enables you to return to normal activities with a pain-free knee and hip. Bacteriophage Therapy

Bacteriophages areviruses that infect bacterial cells and destroy them. They replicate in bacteria and can not infect or destroy human cell. This feature of bacteriophages led to the idea of using them in treatment of bacterial infections. Especially, it is considered a promising approach against antibiotic resistance.

According to theWorld Health Organization (WHO), antibiotic resistance is one of the biggest threats to global health. So, bacteriophage therapy provides an alternative and effective method to antibiotics.

Our center providesan effective treatment solution with bacteriophage therapy  for patientswith infected wounds, diabetic foot wounds, osteomyelitis, and orthopedic implant-relatedinfections. It has been observed that in many severe bone infections,successful results can be achieved within several weeks with bacteriophage treatment. In addition, the most significant feature of bacteriophages is that phages are specific to bacteria, not human cells. So far, no side effects have been identified for bacteriophages.

Oncology

After a diagnosis, we develop individualized treatment plans for every patient in an effort to contain the disease's progression while, ideally, curing them completely.

All stages of cancer are covered by medical oncology, along with a wide range of other areas like management, early diagnosis, patient care, and disease prevention. At this time, assistance is also received from a number of fields, including surgery and radiology.

The method of giving medicine depends on the type of cancer being treated. Adjuvant therapy aims to lessen the risk of a disease recurrence. Neoadjuvant therapy, on the other hand, aims to either safeguard the organ or ready the mass for surgery. When the condition is advanced, it is used palliatively to improve quality of life and lengthen lifespan. Either a single approach or a combination of approaches can be used to complete the task.

Among the methods employed in medical oncology include hormonal therapy, immunological therapy, chemotherapy, and targeted biological therapy. The most common method is oral administration. This approach is used in cancers like breast and prostate cancer that are hormonally sensitive. In order to aid the body in locating and eliminating cancer cells, immunotherapy makes use of molecules that the immune system naturally creates. Chemotherapy employs drugs to halt the growth and proliferation of cancerous cells in an effort to harm them. Importantly, targets that are only present in cancer cells are found in targeted biological therapies. This approach is used to treat a variety of cancers, including breast and bowel cancer.

Cardiology

Conventional Coronary Artery Bypass Surgery

Chest discomfort and potentially fatal conditions like heart attacks occur when the coronary arteries, which supply the heart with blood, become narrowed or blocked. This restricts blood flow and hampers the heart's normal functioning.

Coronary bypass surgery is performed to address blocked coronary arteries. The procedure involves using the patient's own veins, such as leg veins, forearm arteries, or chest arteries, which are prepared and redirected to restore blood circulation without impairing their original function.

Coronary bypass surgery is preferred in the following cases:

            ·  Various vascular conditions

            ·  Heart valve conditions requiring surgery (balloon and stenting)

            ·  Inability to open multiple coronary arteries without surgery

            ·  Coexisting cardiac issues requiring surgery

            ·  Recurrence of vein blockage despite non-surgical treatment

After the surgery, the patient typically stays in the hospital for four to seven days. Once discharged, the patient needs to remain in Turkey for an additional seven days for follow-up appointments. During the initial recovery phase, the patient should primarily rest at home but can engage in light activities. It takes about a month for the patient to return to their normal routine and six months for full recovery and resumption of all activities.

Important considerations:   

            ·  Smoking cessation is crucial.

            ·  Avoid strenuous exercises.

            ·  Follow a strict diet under the supervision of a doctor to achieve weight loss.

            ·  Maintain a regular sleep schedule.

Minimal Invasive Coronary Artery Bypass Surgery

Minimally invasive coronary bypass procedures offer several advantages compared to traditional bypass surgery. Instead of making a bone incision, the heart is accessed through a 7 cm incision below the patient's left breast, between the ribs. This technique allows bypass surgery to be performed on the functioning heart, with the mammary artery being directly visualized and surgically removed. The recovery time is shorter than the conventional approach.

To be eligible for this technique, the patient should not have a history of lung disease, as lung adhesions can impede access to the heart and mammary artery. Overweight patients may also encounter similar challenges. The decision to use this treatment depends on the number and location of the patient's diseased vessels within the heart.

Heart Valve Surgery

Valve repair or replacement becomes necessary when the structure or function of one or more heart valves is compromised, affecting blood pumping. Untreated valve dysfunction can lead to significant heart issues. Valve repair preserves the patient's own tissues, offering the greatest benefit.

Valve surgery is performed under anesthesia, with the heart-lung machine monitoring the patient. The procedure can be done through a breastbone incision or a thoracotomy between the ribs, depending on the patient's anatomy.

After heart valve surgery, there is a 6- to 8-week recovery period. No specific diet is required in the first month. Patients should continue taking prescribed medications, practice proper breathing techniques, and wear compression stockings. Diabetic patients need to be cautious during wound healing. A follow-up appointment with a cardiologist is recommended after one month.

Minimally Invasive Aortic and Mitral Valve Surgery

The mitral valve regulates blood circulation on the left side of the heart. Rheumatic disorders often cause damage to this valve, leading to stiffness and impaired movement. Late-stage complications include stenosis and insufficiency, with symptoms such as difficulty breathing during physical exertion. Prompt intervention is crucial to prevent heart failure.

Minimally invasive mitral valve surgery involves small incisions instead of sternum bone separation, expediting the complex procedure. Patients with a tear in the major artery leaving the heart are not suitable for this technique.

Patients are typically discharged 3-4 days after minimally invasive mitral valve surgery. It takes about 1-2 weeks to return to normal activities. After surgery, there is less need for dressings, and patients can sleep in any position. They also experience reduced discomfort and do not require chest covering.

Aortic Dissection and Aortic Aneurysm Surgery

Aortic dissection occurs when the layers of the aortic artery wall tear, resulting in bleeding. It can happen in patients with or without an aneurysm. Symptoms may include sudden sharp chest or back pain, although some cases are asymptomatic.

Timely intervention is crucial in treating aortic dissection due to its rapid onset and potential harm to the body. Prompt and appropriate surgery is necessary for patients with aortic dissection.

Aortic aneurysms involve the enlargement of the aorta for various reasons. Surgery, such as thoracoabdominal aortic aneurysm correction, is performed based on the location, size, and condition of the aortic valve. Different surgical options include aortic arch correction, bentall surgery, and abdominal aortic aneurysm repair.

Endovascular Aortic Interventions (EVAR - TEVAR)

Endovascular interventions are minimally invasive procedures performed through major blood arteries to access various body areas. Compared to open surgery, endovascular surgery offers benefits such as faster recovery, local anesthesia, smaller incisions, and reduced strain on the heart, making it suitable for patients who cannot undergo open surgery.

One specific technique called EVAR is used for intra-abdominal ballooning. A small tube is inserted through the groin arteries to expand a closed artificial vessel beneath the renal vessels. This eliminates the risk of rupture and may require emergency surgery in rare cases. Proper follow-up care, including CT and ultrasound monitoring for leakage, is crucial.

TEVAR is a similar procedure used for treating chest inflation. A tubular device is inserted through the groin arteries, expanding the closed artificial vessel like an umbrella during the ballooning process.

Total Arterial Revascularization

Revascularization refers to redirecting blood flow in arteries at risk of severe coronary artery blockage through angioplasty and stenting or coronary bypass surgery.

Revascularization therapy is recommended in the following cases:

  ·  Patients with unstable angina or non-ST elevation myocardial infarction (NSTEMI) who do not respond well to intensive medication treatment, as well as patients with acute ST elevation myocardial infarction (STEMI).

            ·  Patients with stable angina who cannot tolerate medications or have uncontrolled angina despite medication use.

         ·  The condition of the patients' coronary arteries plays a role in the decision for revascularization, especially in cases of severe stenosis in one or more major coronary arteries, such as substantial stenosis of the left main coronary artery (LMCA).

Atrial Fibrillation

Atrial fibrillation is a common rhythm abnormality that increases the risk of structural heart issues such as rheumatic heart disease, hypertension, and heart failure. It can cause symptoms that lower the quality of life and lead to systemic embolism, hemodynamic dysfunction, and tachycardia-related myopathy.

Medications are often used to manage atrial fibrillation, including blood thinners to reduce clotting and stroke risk, rate-control drugs, and drugs to restore normal heart rhythm. Surgical intervention may be considered if medication therapy is ineffective or if symptoms are severe and persistent. Surgical procedures for atrial fibrillation include:

            ·        Electro-cardioversion:

            If medications fail or have side effects, shock waves can be delivered to the heart through a painless procedure called electro-
         cardioversion to reset the heart rhythm and restore sinus rhythm.

            ·        Pacemaker:

            When other treatments are ineffective, a pacemaker may be surgically implanted to regulate the heartbeat and maintain sinus
         rhythm.

            ·       Catheter ablation:

           This technique involves using radio waves sent through a catheter inserted into a blood artery to remove the tissue causing
         the arrhythmia and treat the condition.

            ·       Maze procedure:

           Open-heart surgery is required for this technique, which involves making incisions in the chamber responsible for atrial
         fibrillation to prevent abnormal electrical impulses and restore normal heart rhythm.
